The first document, titled “Preparing for your DOT Medical Exam” provides a comprehensive list of items you should know heading into your exam, along with helpful tips so you can prepare efficiently. The second document, the “Medical Examination Report Form” will need to be filled out by you and sent to us so prior to your exam.
Once this form is filled out, you can fax it to our offices (208-232-1006) or send it to info@westfamilymedicine.com. We’ll be sure to save it to speed up the process once you arrive for your exam!
